NSU 2017 Flu Vaccine Schedule
It is that time of year again! The Office of Human Resources is pleased to collaborate with the NSU Pharmacy and ICUBA to provide free seasonal flu shots to employees. This year there are four convenient options for employees to get their flu shots. You do not have to participate in the NSU/ICUBA medical plan in order to receive a free flu shot.
Option 1: On-site Clinics
Free seasonal flu shots are available to all NSU employees at our on-site clinic locations. Employees do not need to be covered under the NSU/ICUBA medical plan but if you are covered under the university’s plan bring your Optum Rx prescription ID card. Click here to complete the registration form VAR-Registration Fluzone UI815AA2017-2018 – BLANK. Bring the completed form to the clinic date of your choice.
October 27
12:00 to 4:00
University School
Wolf Family Grand Lobby
(Center for the Arts)
Option 2: NSU Employee Health Fair, Wednesday, October 25, 2017 from 10 am to 2 pm
Free seasonal flu shots are available to all NSU employees at the NSU Pharmacy table during the annual health fair. Employees do not need to be covered under the NSU/ICUBA medical plan but if you are covered under the university’s plan bring your Optum Rx prescription ID card. Registration is required to receive your flu shot and can be completed online or by faxing a copy of the registration form to the NSU Pharmacy at 954-262-3865.
Option 3: Community Walgreens Location
For employees covered under the NSU/ICUBA medical plan and who are unable to attend one of the on-site flu shot clinics, the annual health fair, or would like to receive a flu shot prior to one of our scheduled events, can obtain a flu shot at a participating Walgreens location (Take Care Clinics are excluded) using your Optum Rx prescription ID card. Walgreens accepts appointments and walk-ins for the flu shot. To view participating Walgreens locations, visit:

Option 4: Physician’s Office Visit
Employees covered under the NSU/ICUBA medical plan may also obtain a flu shot from an in-network physician. The flu shot is covered at 100% but your healthcare provider may charge for an office visit to administer the vaccine.
As a reminder, employees can schedule an immunization appointment Monday through Friday 9 AM 5PM by calling the pharmacy at 954-262-4550, if the clinic dates/times are not convenient.
For more information regarding the flu vaccine, please call the NSU Pharmacy at 954-262-4550.
