NSU Employee Health Fair, Oct. 29
The Annual Health and Wellness Fair is fast approaching! It’s scheduled for Wednesday, October 29, 2014.
If you are enrolled with the NSU ICUBA health plan, you can sign up for a Wellness Screening* offered by Florida Blue. Remember by completing this Wellness Screening you will earn up to 200 points on your Blue Rewards account. Scheduling your appointment is quick and easy: just follow the steps listed below.
STEP 1 – SCHEDULE APPOINTMENT (Employees Only)
1. Click here to make appointment!
2. Enter your email address and create a password (write down password).
3. Select an appointment time and enter your work telephone/extension number.
4. You will receive a confirmation e-mail with the date, time, and location of the event.
*****Registration for an appointment time closes 48 hours prior to October 29th.
Once you have your appointment:
STEP 2 – WELLNESS EVENT
1. Check-in with our Better You from Blue staff at your confirmed appointment time.
2. You will receive a health assessment questionnaire, a #2 pencil, and a screening for blood pressure, cholesterol, and glucose (sugar).
3. Remember to fast at least two hours prior to your appointment time. It is recommended, not mandatory.
4. For Flu Shots be sure to bring your Catamaran Prescription Card.
OBTAIN YOUR RESULTS
1. Complete your Health Assessment questionnaire and return this to the Better You from Blue staff.
2. You will receive a personal and confidential Wellness Report based on your questionnaire and screening results.
CONFER WITH THE HEALTH COACH
• To go over your results and personal health report.
*****Chair massages will be offered to all employees who complete their Health Assessment, so don’t forget to make you appointment!.
